If I am interested in pursuing a healthcare scholarship
opportunity, what is my first step?
Interested students should contact Community Memorial’s Human
Resources Department at 878-7630 and request an application form. Or
request an application form at a job fair that CMH is attending.
What qualifications is CMH looking for in a scholarship
recipient and who determines who will be awarded a scholarship?
Applicants will be awarded a scholarship opportunity based on the
following criteria:
- Completion of the application form;
- Responses to the application’s questions;
- Academic and/or employment references;
- Must have successfully completed the first year of an accredited
program in an approved healthcare program, averaging a 3.0 or better
(B average or better) and must be in good standing;
- Must be willing to complete a portion of an approved internship;
- Must be willing to begin their employment in the career that they
were awarded their scholarship for within two (2) months after the
completion school.
What is the final selection step toward a scholarship?
Candidates will be selected via their application and will be
interviewed by a CMH selection committee. Reference checks will be
done on each candidate that will include their clinical instructors.
If selected for this scholarship, the tuition payments will be made
directly to the college.
How does an applicant know what health care education programs are
eligible for a scholarship opportunity?
The healthcare careers that are or will be eligible for a
scholarship opportunity will be determined on CMH’s technical health
care employment needs and the general job market. Contact the Human
Resources Department at 878-7630 for more information.
What commitment do I have to make to CMH, if I accept the
scholarship?
The scholarship is structured as a forgivable loan. As long as a
scholarship recipient continues their employment in good standing
with CMH, CMH will forgive 25% of their loan for each six months of
employment. Employment beyond 24 months will be dependent upon CMH
open job positions and satisfactory job performance review(s).
If the student leaves the employment prior to the completion of 24
months, the student will be in default of the Healthcare Scholarship
Loan Forgiveness Agreement and, at the discretion of CMH, will be
required to reimburse any remaining unforgiven scholarship amount.
What if there is no job immediately available at CMH, once I
complete my education?
In the event that there is not an open job position immediately
available at completion date, CMH may offer employment in another
position that the student is qualified for. Salary will be
appropriate to the position with credit given for experience. Once
employed, the student is expected to comply with the guidelines
defined in the CMH Employee Handbook

Healthcare Scholarship Loan Forgiveness Agreement
I. Note to Scholarship Applicants:
Community Memorial Hospital Association (CMH) may make available a
healthcare education scholarship that is structured as a forgivable
loan to post-secondary students in good standing, who are seeking a
degree, licensure, or certification in a healthcare related
education program. To be eligible for consideration, applicants are
responsible for reading and following the scholarship criteria and
meeting the application requirements. Community Memorial Hospital
reserves the right in determining an education scholarship recipient
and/or what programs or courses qualify.
II. Scholarship Participation / Eligibility Criteria:
- Must have successfully completed the first year of an accredited
program, averaging a 3.0 or better (B average or better) and must be
in good standing.
- The School’s official course outline must accompany the
scholarship’s application.
- Must be willing to fully disclose to CMH any other scholarship
and/or grant monies being received for the healthcare education
program enrolled in. Students receiving partial scholarship and/or
grant monies may be eligible to receive a partial education
scholarship from CMH; however, the awarded scholarship dollars will
not exceed the healthcare education program’s cost for tuition and
books.
- The scholarship recipient must agree and be willing to:
- complete a portion of an approved internship;
- begin employment in the healthcare discipline that the
scholarship dollars were put towards, within two (2) months after
the completion of the education program.
The scholarship application must be received by CMH to the
attention of the Human Resources Department, no later than three
weeks prior to school registration.
III. Scholarship Disbursement:
1. Only courses taken for credit (and following #2’s criteria above)
shall be eligible for scholarship disbursement. 2. Scholarship disbursements will occur with the student submitting
an official copy of their semester’s transcript and receipts for the
coursework’s required books. Disbursements will be paid directly to
the school. 3. A minimum 2.5 grade point average (based on a 4.0 system) must be
maintained.
IV. Scholarship Loan Forgiveness Agreement:
- At or before the time the
scholarship is disbursed, the student shall be required to sign a
Scholarship Loan Forgiveness Agreement.
- If all criteria and requirements
of Sections I & II are met above, CMH shall forgive the
scholarship loan according to the following schedule:
- twenty-five percent (25%) six
(6) months of continuous employment in good standing with CMH
- an additional twenty-five
percent (25%), twelve (12) months of continuous employment in
good standing with CMH;
- an additional twenty-five
percent (25%) eighteen (18) months of continuous employment in
good standing with CMH;
- the final twenty-five percent
(25%) twenty four (24) months of continuous employment in good
standing with CMH;
V. Scholarship Payback
If the scholarship recipient fails to meet any or all of the
eligibility or participation criteria, or if the employee ceases to
work for CMH at any time for any reason prior to twenty four (24)
months after completion of the course work, the scholarship
recipient shall become immediately liable to CMH for repayment of
the remaining balance on the scholarship loan, together with eight
percent (8.00%) simple interest per annum on the remaining balance.
These amounts may be deducted from the employee’s wages. The
employee agrees to pay all costs of collecting the amounts owed,
including reasonable attorney’s fees, court costs, and other
expenses.
