Diabetes Education Program At Community Memorial Hospital (CMH), we empower patients to successfully manage their diabetes by teaching patients the skills needed to keep their blood glucose levels in their target ranges. Our supportive team will provide management skills and motivational strategies to promote healthy lifestyle changes and prevent complications associated with diabetes. Individualized plans for patients are based upon the American Diabetes Association standard of care and the American Association of Diabetes Educators.  The CMH program has been awarded the prestigious American Diabetes Association Education Recognition Certificate for a quality diabetes self management education program.
