CMH offers a high quality patient experience with friendly care and attention that can only come from a community hospital. Nestled in a relaxing northwoods setting with easy parking for you, your family, and friends, CMH provides state-of-the-art medical technology, a highly trained professional staff, and a modern facility that will make you feel comfortable and cared for.
Visa, MasterCard, Discover and American Express cards are accepted. You can pay your bill online here.
If you have no insurance coverage, we require a deposit prior to or at the time of registration. The balance must be paid in full within 10 days of receiving the itemized bill. Patients without healthcare coverage will receive a discount on total charges. Other payment arrangements will be considered if you call Patient Accounts at 218-878-7033 and discuss them with us.
The CMH Community Care Program is available to individuals who are uninsured and unable to pay for their medical services. Eligibility is determined by measuring family income and assets against current guidelines. Some services may not be eligible under this program. Download the Community Care Program packet which includes the guidelines for assistance, description of program, and application, or contact our Patient Accounts at 218-878-7033 for more information.
We ask your cooperation in furnishing all of the needed insurance information prior to registration. If you have insurance coverage, we will submit a claim to your medical plan for payment of the services. Any balance not covered by insurance is the responsibility of the patient. If insurance payments have not been received within 45 days of billing, payment of the account becomes the responsibility of the patient or guarantor.
You are responsible for providing any information requested by your insurance carrier for prompt payment of the claim. If you choose not to provide additional information, you will be responsible for payment of the balance.
If you are covered under Minnesota Care, Medical Assistance, Medicare, or a replacement plan, a current identification card will be requested at the time of registration. If you are eligible for Contract health services, you must present the referral form or card at registration.
Any hospital expense incurred as the result of a vehicle accident, public liability or workers’ compensation will be considered the personal responsibility of the patient. We do bill insurance and will do what we can to help you collect the claims. However, if the insurance claim is denied or payment delayed, the account becomes self-pay and is due in full.
CMH social workers’ services include discharge planning, depression screening, cognition screening, monitor adjustment issues, information on health care directives, emotional support and referrals to community resources.
